Primary Purpose

Assesses developmental, behavioral and mental health needs. Provides interventions (individual and group) to reduce symptoms and connects to resources.

 

 

Essential Functions

•

Perform clinical surveillance primary and secondary level assessments to assess developmental, behavioral or mental health needs.

•

Develops and implements treatment plan and prioritizes goals.

•

Serves as information and referral specialist. Refers patients to resources to facilitate access to services needed. Services may include: basic necessities, children’s programs, domestic violence resources and other services.  Maintains referral lists. Performs case management for patients who need primary care services.

•

Provides ongoing counseling and psycho-educational support to patients. Provides crisis services by managing mental health emergencies when they arise

•

Documents activities. Completes assessments, follow up notes, and phone notes within specified deadlines. Completes inquiry reports to obtain monthly statistics and submits to assigned Physician. Verifies demographic information and patient information for clients. Monitors and responds to referrals and flags. May manage billing information.

Trains and provides consultation for social work students, nursing students and any other discipline seeking education of services. Provides clinical supervision for social work students

Position(s) may perform other duties in addition to the above as assigned by management.

 


Additional Responsibilities

•

Regularly participates in county and community meetings. This helps to stay connected and updated in order to improve patient referrals to community providers.


*IMPORTANT* In the event of an emergency, as determined by the County Manager or designee, participation in preparedness and response operations should be expected. Employee may be required to fill a temporary assignment in a role different from standard duties, work hours and/or work location in preparation for, during and after the emergency. Employee may also be required to participate in relevant exercises and regular preparedness training.

Thorough knowledge of social work principles, techniques, and practices, and their application to complex casework.

Considerable knowledge of a wide range of behavior and psychosocial problems and their diagnosis and treatment.

Considerable knowledge of family and group dynamics and a range of intervention techniques, as well as principles of casework and training.

•

Ability to formulate appropriate goals and interventions.

•

Considerable knowledge of treatment planning
