Primary Purpose

Provides strategic planning, direction of programs, the day-to-day operations of Live Well Wake and all activities consistent with the goals, objectives and strategic priorities.  Oversees fund availability for Live Well Wake initiatives and approves expenditures from the leadership team as appropriate. Serves in various roles on agency and community-wide committees and task forces to provide expertise, knowledge, or service to help accomplish the desired outcomes and performance measures.

Works with community partners and customers to facilitate successful connections and contributions to identified agency and community outcomes or initiatives

Assists through project management through community partners and customers with establishment of project framework, identify necessary resources for planning/communication/implementation and coordinate with other county departments as necessary.

Supports and coordinates Community Health Needs Assessment every three years

•

Coordinates with other relevant stakeholders, assist with identification and use of critical data, assist with planning for reporting of results.

Position(s) may perform other duties in addition to the above as assigned by management.

 

*IMPORTANT* In the event of an emergency, as determined by the County Manager or designee, participation in preparedness and response operations should be expected. Employee may be required to fill a temporary assignment in a role different from standard duties, work hours and/or work location in preparation for, during and after the emergency. Employee may also be required to participate in relevant exercises and regular preparedness training.

Knowledge and Experience

•

Requires deep knowledge of discipline typically obtained through advanced education combined with extensive experience. Typically viewed as having a specialty within a discipline. May have broad knowledge of project management.

•

Within Medical or Scientific Disciplines, typically requires a doctoral degree and post-doctoral training.


Qualifications


Required Education

Bachelor's degree in Human Services, Health or related field

Five years of job-related experience
