Provide nursing care management and other specialty support services to clients who are experiencing homeless, formerly homeless and/or periodically involved in the legal system. Nursing skills and activities according to professional standards, including assessment, outcome identification, service planning and implementation, counseling, psychobiological interventions, injections, medication management, health coaching and self-care skills, care management, and health promotion/health maintenance. Delivers Nursing Care by developing a nursing diagnosis; establishing nursing care objectives; defining nursing interventions; noting changing conditions and needs; re-evaluating care objectives. | | | | | | | | |
