Primary Purpose

Provides comprehensive and complex case management for court involved participants by coordinating services which includes substance use disorder treatment, mental health treatment, transportation, housing, child and family issues, life skills, employment, and housing assistance.  Performs assessments on candidates to determine eligibility and completion of intake interviews on new participants, completes grant required surveys, observes urine and oral fluid collection for drug testing, advocates for needed services, attends court sessions and works closely with the multi-disciplinary team to assist in decision making.

 

Essential Functions

Performs case management for a caseload of at least 35 participants, encourages participant progress, addresses concerns, utilizes problem-solving skills and provides crisis intervention.

Participates as a member of the multidisciplinary team and serve as the primary liaison to agency partners, facilitates team staffings, communicates participant concerns and information with staff and team members frequently.

Coordinates weekly drug testing schedule and conducts drug screens on recovery court participants with collaboration with the Division of Public Safety (probation).

Assesses and interviews candidate referrals to determine program eligibility.

•

Maintains participant records and progress notes in database system and compiles data when requested. Documents attendance, drug screen results, treatment notes, in database system.

Monitors attendance for substance use disorder treatment and recovery support meetings.

•

Works with the Wake County Drug Overdose Prevention Initiative and Public Health/Health Promotion Chronic Disease Prevention Section partners and staff as requested by Division leadership.

*IMPORTANT* In the event of an emergency, as determined by the County Manager or designee, participation in preparedness and response operations should be expected. Employee may be required to fill a temporary assignment in a role different from standard duties, work hours and/or work location in preparation for, during and after the emergency. Employee may also be required to participate in relevant exercises and regular preparedness training.

Bachelor's degree in Social Work or related field
