Essential Functions | | | Priority | % Time | • | Assesses family's well-being needs with child welfare social worker. Completes an assessment of the environmental impacts to family well-being. Compares results with normal values and findings. Determines if additional information is needed. Triages individual symptoms to assist with child welfare social worker with identifying well-being needs for family. Determines need for referral to provider. Identifies patterns of cause and effect related to client’s health status, including relevant factors in the family and community. Reassesses as indicated and monitor for changes in family well-being status. Participates in in home visiting with or without social worker from child welfare. Assesses the family's environment and its impact on health and well-being. Participates in Child and Family team meetings and other meetings as needed with Child Welfare, medical providers, community agencies and internal team meetings. Participates in medical home and sub specialty care appointments with families as needed. Potential to have court involvement with Child Welfare cases. | 1 | 30% | • | Plans and implements well-being recommendations for families alongside the child welfare social worker. Initiates or participates in multidisciplinary planning when appropriate. Evaluates and modifies the child welfare plan as indicated based on family's response to interventions, changes in well-being or alteration of family goals with child welfare. Communicates new information to others involved in the family's care as appropriate. | 2 | 30% | • | Teaches and counsels client and significant others regarding the family's well- being status for the purpose of increased knowledge to promote optimum level of well-being functioning, participation in self care and ability to make informed decisions.
