Primary Purpose

Provides quality nursing services in outpatient clinical settings to prevent illness, promote optimal physical and social health and protect individuals and communities from potential harm.

 

 

Essential Functions

Assesses client's physical, mental and social health status. Administers standardized screening tools as indicated and draws conclusions for referrals based on results. Compares client results with normal values and findings. Determines if additional information is needed. Completes procedures and medical orders as requested by the Provider. Triages individual symptoms to provide guidance to client or caretaker based on approved Standing Orders or approved Medical Guidelines.  Determines need for referral to provider. Identifies patterns of cause and effect related to client’s health status, including relevant factors in the family and community. Reassesses as indicated and monitor for changes in health status.

Plans and implements nursing care activities based on client needs and resources available. Initiates or participates in multidisciplinary planning when appropriate.  Verifies competencies of Licensed Practical Nurses (LPNs) and unlicensed assistive personnel (UAP) that will perform delegated nursing activities. Evaluates and modifies the plan of care as indicated based on client's response to interventions, changes in health status or alteration of client goals. Communicates new information to others involved in the client's care as appropriate.

Teaches and counsels client and significant others regarding the client's health care status for the purpose of increased knowledge to promote optimum level of health functioning, participation in self care and ability to make informed decisions. Adapts content and teaching method as appropriate for the learner's needs, abilities and knowledge level. Refers to additional resources as indicated by response to teaching and identified needs.

Determines need for immunizations based on Immunization Policy and State Guidelines. Provides anticipatory guidance regarding vaccine efficacy and necessity.  Promotes all vaccines as indicated by client need or state mandates. Records all vaccine information in the record according to policy and procedures.

Documents all pertinent client information on the appropriate record in accordance with policy, procedure, and regulations; completes within a time period consistent with the client's need for care. Describes accurately all aspects of nursing care including assessment, planning, implementation and evaluation; reflects the communication of client information to other persons. Reports relevant information to other persons responsible for, or involved in, the care of the client to facilitate collaborative continuity of care.

Additional Responsibilities

•

Ensures own knowledge and understanding of the state and county regulations governing nursing practice and relevant specialty areas. Obtains required continuing professional education and maintains required current licensure and certifications.

Participates in quality assessment and improvement activities.

•

Accepts additional duties as assigned to meet the work area goals and/or Wake County Human Services goals.


*IMPORTANT* In the event of an emergency, as determined by the County Manager or designee, participation in preparedness and response operations should be expected. Employee may be required to fill a temporary assignment in a role different from standard duties, work hours and/or work location in preparation for, during and after the emergency. Employee may also be required to participate in relevant exercises and regular preparedness training.

Knowledge, Skills and Abilities

•

Conducts client or parent/guardian interview to obtain medical/developmental/social/environmental history, current health concerns, symptoms, and goals.

•

Performs physical examination within the Registered Nurse scope of practice and specialty certifications.

•

Reads, analyzes and interprets medical records, immunization records, standardized screening tools and related documentation. Synthesizes with nursing knowledge to form a plan of care based on subjective and objective information to reach positive client outcomes.

•

Performs, teaches and delegates nursing and medical procedures in accordance with NC Board of Nursing guidelines and medical provider orders.

•

Documents clients records in paper and electronic systems. Participates in periodic audit and review of client records for quality assurance.

•

Communicates with clients, colleagues and others through multiple channels including but not limited to email, video conferencing and telehealth platforms. Ability to communicate with clients from various cultures, backgrounds, belief systems and languages when interpreters are available.

•

Contributes nursing knowledge and expertise as member of interdisciplinary teams; advocate for needed services and entitlements that promote and protect the health of a client and/or specific population.
