Essential Functions | | | Priority | % Time | • | Places children in a timely manner, in the least restrictive environment possible, that meets the child's needs. Identify placements that afford youth the opportunity to remain in their community and school of origin whenever possible and appropriate. Make every effort to match the child to an appropriate placement and lessen the likelihood of disruption and further trauma. | 1 | 70% | • | Serve as the Afterhours/Weekend on call Placement Social Worker on a rotating basis. | 2 | 10% | • | Complete documentation and tracking (Daysheets, Case Notes, etc.) in timely manner in accordance with policies and procedures. | 3 | 10% | • | Partner with foster care and congregate care agencies to complete monitoring of service delivery. | 4 | 10% | | | | | | | |
