Primary Purpose

Facilitates specialized group meetings, which may involve family members served by the agency and their supports, internal staff, and external stakeholders to create, implement, and update a plan that builds on the strengths and addresses needs.  Works collaboratively with direct care staff to schedule and evaluate meetings.

Organizes and facilitates specialized meetings - coordinates physical or virtual room set up, confirms date and time of meeting with internal staff and stakeholders (family, partners, etc), manages documentation of attendance and key action items, keeps the participants focused on common task using best practice model for meeting, and ensures adherence to protocols and policies.

Collects and enter data from meetings into identified databases by deadlines - assures accuracy of data entered.

•

Makes recommendations to Supervisor to improve caseworker and supervisor practice, including revisions or additions to division protocols.  Gathers feedback and communicate with case workers and Supervisor to identify areas of strength and need for improvement.

•

Participates in webinars, training, and/or committee work, along with Child and Family Services Reviews (CFSR) using the Onsite Review Instrument (OSRI).  Supports activities related to the implementation and operationalization of new policies, practices, and systems, as requested. Conducts other qualitative reviews and analyses, as requested.

Position(s) may perform other duties in addition to the above as assigned by management.

 

*IMPORTANT* In the event of an emergency, as determined by the County Manager or designee, participation in preparedness and response operations should be expected. Employee may be required to fill a temporary assignment in a role different from standard duties, work hours and/or work location in preparation for, during and after the emergency. Employee may also be required to participate in relevant exercises and regular preparedness training.

Promote and support the consensus of safety plans

•

Excellent communication skills - ability to convey complex concepts
