Essential Functions | | | Priority | % Time | • | Provides Special Assistance In Home Program case management services and administers funding for the health and safety needs to disabled and Private Living Medicaid eligible consumers in the community at risk of adult care home or mental health group home placement. | 1 | 70% | • | Provides long term or short term case management services to elderly and disabled consumers in the community with brittle support systems needing assistance to meet their needs. | 2 | 15% | • | Serves as Targeted Housing Program Referral Agency lead point person by assisting consumers or advocates helping consumers meeting program criteria to get access to participating safe and affordable housing complexes. | 3 | 5% | • | Completes all required and recommended training | 4 | 5% | • | Completes all state, county and court documentation as required including time sheets and day sheets. | 5 | 5% | | | | | | | |

Additional Responsibilities | • | Assists Wake County Services Intake with getting Low Income Energy Assistance Program applications signed by homebound consumers. | | | |
