Primary Purpose

This position provides primary management and supervision for the Housing Resource Team which provides stabilization services to help prevent citizens from experiencing homelessness either working to help them remain in their housing or relocating households when remaining in the present location is not possible.  The team is comprised of a Housing Navigators, Diversion Specialists and four Social Workers.  This position oversees client and program services, data collection, financial/budget (check requests/encumbrances/contracts) summaries, and documentation needed per program requirements.

Implements Wake County’s Homeless Prevention Program to engage and address the unmet needs of our citizens who are experiencing a housing crisis and are at imminent risk of losing their housing, becoming homelessness and are presently disconnected from and unengaged in community services, and provide iterative continuous quality improvements; implement and enforce policies, procedures, and a standard of array of services that will benefit the target population-- regularly review and update all policies, procedures and standards; Research best practices and develop standard protocols for engagement and trust building; Develop a robust network of community providers who will offer services and accept referrals; Develop forms, assessments and tools required to implement strategies and connect clients to services; review regularly and update all forms.

Supervises a team of social workers work; Posts vacancies, interviews, proposes for hire, and trains new staff; Provides ongoing coaching and consultation to staff regarding service provision, initiates corrective action as needed, and ensures each staff receives ongoing skills training and professional development; Provides supervision to staff during weekly team meetings, weekly individual supervision sessions, and any other time deemed necessary; Conducts periodic performance reviews, including quarterly evaluations and work plans; Conducts periodic review of team’s personnel needs and structure for quality assurance; Reviews client records for quality assurance and proper maintenance; Ensures appropriate use of technology by case management team for proper record maintenance and efficiency; Facilitates weekly Wake Prevent! Team Meeting; Manages caseload assignments; Maintains availability to clients for consultation as requested; Ensures quality of overall provision of services by the Team; Completes other tasks associated with administration and supervision.

Operationalize program and ensure core principles as described below are followed:

a. Crisis resolution responses must include: rapid assessment and triaging, based upon urgency; an instant focus on personal safety as the priority; de-escalation of the person’s emotional reaction; definite action steps the individual can successfully achieve; assistance with actions the individual is temporarily unable or unwilling to attempt; and returning the person to control over their own problem-solving.

b. Client choice, respect and empowerment with a constant emphasis on the client’s goals, choices, and preferences, an unwavering respect for their strengths, and reinforcement of progress are essential for empowerment. This does not mean clients are protected from the natural consequences of their actions.

c. Provide the minimum assistance necessary for the shortest time possible providing “just enough” to prevent homelessness enables a program to help far more people in crisis. Often this means ensuring resources are used to help persons at-risk of losing housing of any kind—persons who would otherwise end up on the street or in an emergency shelter—before using resources to aid with other needs.

d. Maximize community resources utilizing mainstream assistance programs.

e. Use the right resources at the latest stages of an individual’s housing crisis when it is virtually certain she or he would have become homeless without assistance.

Completes administrative duties and program oversight to include approving and submitting payment requests to ensure timely payment of eligible costs to utility companies and other third parties for rental application fees; security deposits and rents; ensuring compliance with all federal rules and regulations; participates in complaint resolution with landlords, service providers, and clients, elevate to Division Director when necessary; ensure physical and HMIS client files are maintained; develop and manages Housing Resource Team budget; Manages and provides outcomes data and statistics and submit according to scheduled deadlines; attends supervision with Division Director as scheduled; attends other departmental meetings as scheduled; maintains and oversees maintenance of client and staff records, reports, and other required documentation, manages grants and other fiscal information and reports as assigned. Other Administrative activities related to providing general oversight, coordination and reporting on program outcomes.

Implements appropriate staff trainings and continuing education opportunities; Ensures all staff maintain required certifications and credentials; Develops and implement team building activities; Annually creates and oversees implementation with staff of individual Staff Development Plans and Work Plans; Maintains credentials and required certifications, as well as completes annual Staff Development Plan; Completes other tasks associated with Staff Development and Training that arise.

Participate in community outreach activities to include:  developing and maintain relationships with Regional Centers and other community locations were housing resources are integrated into service delivery; developing and maintain relationships with nonprofit and faith partners who provide cash assistance services to citizens who are experiencing a housing crisis; and developing and maintain strong collaborative relationships with hospitals, clinics, behavior health agencies, mainstream services providers to facilitate the connections for persons experiencing housing crises.  Works with partnering agencies on programming initiatives; Attends community/partnership meetings to maintain positive community relationships and representing the Wake Prevent!; Acquires and disseminates knowledge and information to internal staff; Remains knowledgeable of community resources and other services for clients and shares such information with staff and clients; Completes other tasks associated with Community and Partner Relations that arise.

Position(s) may perform other duties in addition to the above as assigned by management.

 

*IMPORTANT* In the event of an emergency, as determined by the County Manager or designee, participation in preparedness and response operations should be expected. Employee may be required to fill a temporary assignment in a role different from standard duties, work hours and/or work location in preparation for, during and after the emergency. Employee may also be required to participate in relevant exercises and regular preparedness training.
